Patient Privacy

San Diego Sports Medicine & Orthopaedic Center is dedicated to maintaining the privacy of your protected health information (PHI). In conducting our business, we will create records regarding you and the treatment and services we provide to you. We are required by law to maintain the confidentiality of health information that identifies you as well as your health status. We are also required by law to provide you with this notice of our legal duties and the privacy practices that we maintain in our practice concerning your PHI. San Diego Sports Medicine & Orthopaedic Center is open from 9:00 a.m. to 5:00 p.m. Our telephone number is (619) 229-3932. You may also reach us by fax at (619) 582-2860. Our telephones are answered 7 days a week, 24 hours a day. If you call us after hours, our answering service will take a message. If there is an emergency, our answering service will contact the on-call physician and relay the message to him/her.

Patient Appointments

In order to provide the best medical care possible, it is important that we have all the tools necessary to do so. We ask that you bring the following information to your appointment with us so that we can appropriately diagnose and treat your orthopedic condition:
  • X-Ray films and reports
  • MRI films and reports
  • Pertinent test results (i.e., nerve conduction tests, etc.)
  • List of previous surgeries, injuries, conditions, or other health problems
  • Insurance card
  • Medicare card (if applicable)
  • Medi-Cal card (if applicable)
  • Picture identification card (i.e., driver's license, etc.)
  • Referral from your primary care provider (if applicable)
  • Name and address of your primary care provider
  • Current medication(s) list (both prescription and non-prescription)
  • HMO Letter of Authorization

Insurance Billing

San Diego Sports Medicine & Orthopaedic Center is contracted with many insurance carriers. We are happy to bill your insurance carrier as a courtesy. We require a copy of your insurance card and a picture identification card prior to services being rendered. If services are denied by your carrier or if you are ineligible to receive benefits, you are financially responsible for all incurred charges. Please be prepared to pay your required co-payment or deductible at the time of service.

Health plan coverage varies significantly by carrier, by employer, and/or by contract. We cannot know the benefits and exclusions of each patient's health plan. It is the patient's responsibility to know and understand his or her health plan coverage and benefits.

Please be prepared to pay for all patient due amounts at time of service. This includes:
  • Co-payments
  • Deductibles
  • Outstanding Balances
For your convenience we accept cash, checks, debit cards, MasterCard, and Visa. If payment is not made at the time of service there will be a $15.00 fee added to your account to cover our costs associated with preparing, sending and processing patient statements.

Thank you for your cooperation and helping us to keep our cost to patients at a minimum.

Referral and Authorizations

If your insurance company requires a referral from your primary care physician (PCP) for an orthopaedic consultation, please make sure you obtain the authorization number referral before calling for your appointment. Our office must be in receipt of the referral from your PCP before booking your appointment. If our office has not yet received your referral, you will be asked to contact your PCP's office to have them fax the referral to us.

Referrals for additional orthopaedic services that your orthopaedic physician may require such as MRI, physical therapy, CT scans, etc., will be ordered by our office. Please allow 5 to 7 working days to process your referral.

Medicare

If you are covered by Medicare (Part B), San Diego Sports Medicine & Orthopaedic Center will bill them for services rendered. We will bill your supplemental insurance upon request. We require a copy of all insurance cards for billing.

Workers' Compensation/Industrial Injuries

San Diego Sports Medicine & Orthopaedic Center works with many industrial medicine entities, including Workers' Compensation, the Department of Labor, and Tribal First. If you are injured on the job or feel that your problem may be work-related, please make sure you have informed your employer. Our Workers' Compensation Coordinator will need to obtain authorization from your employer's Workers' Compensation carrier prior to your visit. If your employer is self insured, we will need an authorization from them that your injury will be covered. If your treatment is denied as being non-work related, you are financially responsible for all incurred charges.

Disability Forms

Please leave completed disability forms with your nurse at the time of service. All disability forms must be accompanied with a filing fee of $25.00 prior to completion. Since your physician must review your medical records and enter information from your medical chart, we require 7 to 10 working days for completion.

Fees

If you have questions regarding our fees, we encourage you to discuss them with our Billing staff. Our fees for professional services are consistent with those in the community. An estimate for proposed services may be obtained upon request, and in many instances are given routinely. We suggest you contact your insurance prior to services being rendered so that you are aware of your potential financial responsibility.

Statements

You will receive a statement from our Billing Department once a month if there is an outstanding balance. The billing statement will itemize services as well as any payments, deductibles, or co-insurance amounts applied by your carrier. If you do not understand your statement or have questions regarding your balance, please feel free to contact our Billing Department for clarification. If your insurance delays processing or processes your claim incorrectly, you will need to contact them directly. If you cannot meet your financial obligation, please contact our Billing Department. Every effort will be made to work out an acceptable payment plan. You will continue to receive a statement until all of your charges and all dates of service are paid in full.

MasterCard and VISA are accepted.

Pre-Surgery Preparation
  • Nothing to eat or drink 8 hours prior to surgery
  • Wear loose-fitting clothing
  • Arrange for a ride home after surgery
  • Arrive 1 to 2 hours before surgery (depending on surgery)
  • Follow the instructions on your physician's instruction sheet
  • Some surgeries require a "pre-surgery" class - check with your nurse
  • Some surgeries require the donation blood - you nurse will assist you in making arrangements to donate your own blood prior to the surgery should you choose to do so.
Post-Op Information

Read the instructions and information packet given to you from the hospital or surgery center.

Depending on your surgery, you may need postoperative physical therapy. If your surgery is outpatient, your physician's nurse will assist in obtaining the necessary referral and authorization for outpatient physical therapy. If your surgery is inpatient, the discharge planner will assist you in making arrangements for outpatient or in-home physical therapy before you are discharged.
