We ask that all new patients complete their health history forms online via our Patient Portal. Otherwise, please arrive 30-45 minutes prior to your appointment time to complete your paperwork. An appointment is required to complete your medical history online.
Disability Forms
Please leave completed disability forms with us at the time of service. All disability forms must be accompanied with a filing fee of $25.00 prior to completion. Since your physician must review your medical records and enter information from your medical chart, we require 7 to 10 working days for completion.
